With much emotion and 30 years of amazing gratitude I am informing you, my primary care patients and families, that I have closed my primary care practice on September 22nd, 2023 to focus moving forward on part-time consulting with a goal of supporting and focusing on the developmental and mental health crisis in our community in our children and youth. I will miss all of the many children and families who have provided me the privilege of being a part of their growing years , you have been family, and know each child and youth is a unique star and I wish good health and amazing life experiences for all.

I recognize that changing physicians may be stressful. As I was unable to procure a primary care physician to assume my practice, I advise patients and families in order to ensure continuity of care to find a new family physician, pediatrician or nurse practitioner through personal enquires or by contacting health care connect at 1-800-445-1822 www.ontario.ca/heatlhcareconnect in support of finding new physicians.

Please note Capital KidZ Medical Centre (Dr. Liddle’s new consulting site) is not accepting new primary care patients.

Patients who have an ongoing complex paediatric condition or identified significant developmental or mental health issues may advise their new physician that Dr. Liddle may offer to remain available (with the prior discussion/knowledge and agreement by Dr. Liddle) to provide care and advice on this particular aspect of their care by consultation only starting October 10, 2023 at Dr. Liddle’s new practice at Capital Kidz.

After September 22, 2023 all patient medical records will be placed in storage. Chart copy may be requested via chart transfer page. For immunization information only you can try reaching out to Ottawa Public Health for immunization information on file.
